I have done both options depending on the enemy and the fiction of the player that unleashed.
This is the reason it is not specified as it allows for the outcome to be a conversation between the player and the MC.
I always explain it to players as a narrative +1 damage, but during game sessions is always something different. A werewolf used the terrible harm to rip a vampires arm out, for example.
in vanilla aw, terrible harm is +1 harm. You could use that as a guide post.
All good answers! I always remind players that it’s a significant escalation, like moving from a knife to a machete or from a handgun to a shotgun.
