Well, remember that the player chooses which box(es) they mark when they take harm – the harm boxes aren’t meant to go in order of severity, aside from the ‘fiction penalty’ -> ‘stat penalty’ -> ‘death’. Still, if it’s standing out to them as odd it’s an easy change.

Ah, no worries! In practice most of the time people will fill them from the top down, but it does have the nice effect that you can choose to trigger your Death Move any time you take harm.
