Tremulus and Mythos World are similar to the extent that they both rely heavily on the conventions established by Apocalypse World and Dungeon World, and they both draw on the classic 1920s setting and the basic character concepts established in the original Call of Cthulhu game. They differe in appoach. Tremulus maintains a sanity point and hit point system and uses lots of roll modifiers as move bonuses. In Mythos World, mental disorders and physical injuries are direct results of moves. Mythos World also favors move results that have specific narrative effects. Tremulus has meatier procedures for developing settings and storylines, including the extensive and interesting Ebon Eves scenario generator. In short, Mythos World is a simpler representaton of Cthulhu roleplaying in the Apocalypse World engine, while Tremulus has deeper rule systems.
